How berberine supplements compare
| Form | What you should know |
|---|---|
| Berberine HCl tablets | A salt form of berberine, with the amount stated on the label. Placebo-controlled trials pooled in a 2025 review used purified berberine tablets or capsules at 300–1,500 mg a day. |
| Daruharidra (Berberis aristata) root powder | A whole-plant ingredient. In one Indian laboratory study, roots contained about 0.8–1.9% berberine, varying with where and when they were collected, so a powder supplies far less berberine, in variable amounts. |
| Dihydroberberine | A modified form. A five-person pilot study, funded by the ingredient's maker, found that 100–200 mg of dihydroberberine gave higher blood levels of berberine than 500 mg of berberine. It was a short absorption study, not a test of health outcomes. |
| Berberine with fenugreek (Pureberb) | Fenugreek (methi) has mostly been studied as whole seed at 5 g or more a day. Pureberb's 100 mg of extract a day is a much smaller amount, and we know of no trial that has tested whether it adds anything to berberine. |
How to take it and when
- Dose: 1 tablet twice a day, 15–30 minutes before meals, with water: one before each of two main meals, not both at once. Do not exceed the recommended daily dose.
- Digestive effects: the most common side effects of berberine are abdominal pain, constipation, diarrhoea, nausea and vomiting. Stop and speak to your doctor if they persist.
- Before judging: trials in people with metabolic conditions ran for 12–20 weeks. Little is known about use beyond about five months, so review continued use with your doctor.
Who should not take it, and who must check with a doctor first
Do not take Pureberb if you are pregnant, trying to conceive or breastfeeding, and never give it to a baby or child. The US National Center for Complementary and Integrative Health (NCCIH) advises against berberine in pregnancy and breastfeeding, and warns that it can cause or worsen jaundice in newborn babies, which could lead to a life-threatening problem called kernicterus. Fenugreek is also not considered safe in pregnancy above food amounts.
Speak to your doctor or pharmacist first if you take any regular medicine. This matters more for berberine than for most supplements:
- In healthy men, taking berberine for two weeks reduced the activity of three liver enzymes (CYP2D6, CYP2C9 and CYP3A4) that break down many common medicines, which could raise the levels of those medicines in the body.
- Berberine may add to the effect of diabetes medicines (raising the risk of low blood sugar) and of blood-pressure medicines.
- Never stop, reduce or replace a prescribed medicine to take berberine.
Also check first if you have a liver or kidney condition or any other diagnosed condition, or have ever reacted to fenugreek, which can cause allergic reactions.
Questions buyers ask
What is berberine HCl?
Berberine hydrochloride, a salt form of berberine. Pureberb's label states the amount as berberine HCl: 500 mg per tablet, 1,000 mg a day at the label dose.
What are the uses and benefits of berberine?
Berberine tablets are sold as food supplements to support normal metabolism alongside a balanced diet and regular activity. Most berberine research has been in people with metabolic conditions, usually under medical care, and those findings are not a guide to what a supplement will do for you. The NCCIH says there is not enough rigorous evidence to say whether berberine helps with weight loss.
Is berberine the same as daruharidra?
No. Daruharidra is a plant that contains a small, variable amount of berberine. A berberine HCl tablet contains a measured amount of berberine itself.
Is berberine a natural alternative to metformin?
No. Pureberb is a food supplement, not a medicine, and it is not a substitute for metformin or any other prescribed treatment. The NCCIH warns against replacing diabetes treatment with an unproven product. If you take metformin or any other diabetes medicine, take berberine only if your doctor agrees.
Can I take Pureberb with other herbal products?
Do not combine it with other berberine supplements or berberine-containing herbs such as daruharidra or goldenseal. Ask your pharmacist before combining it with any other herbal product.
